Have your bank account drafted and be freed from writing checks.  You will still receive a bill and a notice indicating when your bank account will be drafted.  Click above for the authorization form, complete it, and return it to the city along with a voided check.

 Occupational Tax Certificate Application

Please download and complete the application, and then return the application to the address printed on the form.  To expedite this process, make sure you list a phone number so we can reach you to schedule an inspection for a certificate of occupancy, if necessary.
